This assessment is based on real-life skills used in everyday living and has been developed by an Occupational Therapist with over 23 years of experience supporting individuals to build confidence, independence, and life skills.

This is not a test.
There are no right or wrong answers.
Each individual's idea of a “good life” looks different. Your responses simply help build a clearer picture of their current situation — and identify what level of support, structure, or next steps may be most appropriate at this point.
THEIR JOURNEY
A Framework for Understanding Where They’re At
We use four stages to help make sense of current functioning and identify what may be needed next, supporting a more intentional and meaningful Good Life Design.

Emerging Stage
Building safety, awareness, and the foundations of a Good Life — particularly as the individual adjusts to new circumstances, challenges, or changes in their environment.
This may present as a shift from merely surviving day-to-day needs toward beginning to explore what thriving life may look like for them.

Growing Stage
Developing skills, confidence, and consistency in daily life, as the individual continues rebuilding and adapting at their own pace.
This may present as emerging periods of stability or “good days,” although these may not yet be consistent across different environments or situations.

Blooming Stage
Strengthening independence while creating a life that feels more meaningful, sustainable, and aligned with the individual's preferences and goals.
The individual may demonstrate a clearer sense of direction and is building routines and behaviors that support more consistent participation and engagement.

Thriving Stage
Living with increased confidence, clarity, and direction, where the individual's Good Life is more consistent and self-directed.
This may present as maintaining progress, exploring new opportunities, and continuing to grow in meaningful ways.

Some people are just starting.
Some are finding their way again after changes or challenges.
Some are strengthening what they already have.
